These are the forms used in a New York uncontested divorce, in the order they are used, with the current revision from the New York State court system. They are provided for reference. The forms are the mechanical part of the process. The settlement agreement that goes with them is the part that decides what you receive and what you pay, and it is the document to have prepared and reviewed by an attorney. NDA Provision: sample confidentiality clause for use in a settlement agreement.
Notice of Automatic Orders: served with the summons; lists the six restrictions that bind both spouses from the start of the case, including the ten day notice requirement added in January 2026.
Notice of Guideline Maintenance: served with the summons; states the maintenance formula and the current income cap.
Notice Concerning Continuation of Health Care Coverage: served with the summons; explains that a spouse may lose coverage under the other spouse's plan when the divorce is final.
Summons with Notice (UD-1): starts the case where no complaint is filed at the outset.
Summons (UD-1a): starts the case where a verified complaint is filed with it.
Verified Complaint (UD-2): the sworn pleading stating the ground for divorce and the relief requested.
Affidavit of Service (UD-3): proof that the defendant was served.
Sworn Statement of Removal of Barriers to Remarriage (UD-4): required where the marriage was solemnized by a member of the clergy.
Affirmation (Affidavit) of Regularity (UD-5): the attorney's statement that the papers are in order.
Affidavit of Plaintiff (UD-6): the plaintiff's sworn statement of the facts supporting the divorce.
Affidavit of Defendant (UD-7): the defendant's sworn consent to the divorce proceeding without further notice.
Annual Income Worksheet (UD-8(1)): computes each party's income for support purposes.
Maintenance Guidelines Worksheet (UD-8(2)): computes guideline maintenance using the current cap.
Child Support Worksheet (UD-8(3)): computes guideline child support using the current cap.
Support Collection Unit Information Sheet (UD-8a): used where support will be paid through the Support Collection Unit.
Qualified Medical Child Support Order (UD-8b): directs an employer's health plan to cover the children.
Note of Issue (UD-9): certifies that the case is ready for the judge.
Findings of Fact/Conclusions of Law (UD-10): the proposed findings the judge signs.
Judgment of Divorce (UD-11): the proposed judgment the judge signs.
Part 130 Certification (UD-12): the attorney's certification that the papers are not frivolous.
Request for Judicial Intervention (RJI) (UD-13): assigns the case to a judge.
Addendum for the RJI (UCS-840M): the matrimonial supplement to the RJI.
Notice of Entry (UD-14): served with the signed judgment; starts the time to appeal.
Affidavit of Service by Mail of JOD (UD-15): proof that the judgment was served.
Affidavit in Support of Application to Proceed as Poor Person, Poor Person Order, and Affidavit of Service of Proposed Poor Person's Order: used to request a waiver of court fees.
Affidavit in Support of Application to Proceed as Poor Person, Poor Person Order, and Affidavit of Service of Proposed Poor Person's Order: used to request a waiver of court fees.
Postcard Sample: the postcard the court sends when the judgment is signed.
Certificate of Dissolution: filed with the Department of Health to record the divorce.
Notice of Settlement: notice that a proposed judgment will be submitted to the court.

NYS Case Registry Filing Form (Non-IV-D orders): registers the support order with the state.
UCS-111 Child Support Summary Form: summarizes the support terms for the court.
DRL 255 Addendum: the statement that each party has been advised about health insurance coverage after divorce.
Short Form Application for Child Support Services: application to the Support Collection Unit.
Net Worth Statement (PDF): the sworn financial statement each party must complete and exchange before the preliminary conference.
